Restoring Independence at Home: Designing Regional Systems for Sustainable, Replicable Aging-in-Place Models

Session Description
Healthcare systems, housing providers, and aging services organizations face growing pressure to move beyond isolated home modifications. Coordinated, scalable aging-in-place strategies are increasingly being implemented in efforts to meet the needs of the aging population.
This session explores how clinical assessment, environmental risk governance, and cross-sector system design can transform fragmented referrals into integrated regional operating models supporting sustainable aging at home.
Attendees will learn to identify high-risk environmental zones and align functional mobility needs with institutional design and accessibility standards. Participants will also gain guidance on supporting implementation decisions through coordinated clinical, housing, and community leadership that advances aging-in-place initiatives.
The session will provide a practical governance framework for improving outcomes, reducing system strain, and strengthening partnerships across healthcare, housing, and aging services ecosystems. This includes those that serve older adults with complex clinical and environmental needs.
About the Speaker
Dr. Comfort Uwadiae, DPT, CEAS, ECHM, CAPS is the Founder and Managing Member of Opt2Restore LLC, a New York based aging-in-place, home modification consulting and governance firm. She is the creator of the Opt2Restore™ C.A.R.E. Model, a standards-based framework aligning healthcare delivery with housing systems and community partners to support safer care transitions, environmental risk reduction, and long-term independence.
A former physical therapy provider and Clinical Instructor at Montefiore Hospital, Dr. Uwadiae brings over 13 years of experience across acute critical care, inpatient rehabilitation, outpatient services, and home-based care. She maintains active clinical engagement through Catholic Health, supporting transitions from healthcare systems into real-world living environments.
Her work focuses on building scalable, accountable aging-in-place and cross-sector integration systems for aging populations and medically complex patients. She collaborates with healthcare organizations, housing partners, and community coalitions to develop governance structures and implementation pathways for regional aging-in-place initiatives.

Speaker: Michael Splaine
Session Description
An estimated 20% of persons living with Alzheimer’s disease in the US live alone with their thinking skills diminished and facing a progressive disorder. How they fare with their disability may be due to how they either build or participate in a community, how well they plan for their aging in general and the cognitive changes they face and aging in general and adapting their living spaces.
This workshop will highlight some of the data about this unique group, describe resources that NAIPC members can access for their own learning and for clients and list community-based organizations with specific programs for live-alones with AD as well as more general programs that can be a resource for them.
About the Speaker
Michael Splaine, is an internationally recognized leader in health and long-term care policy with over 35 years of experience advancing Alzheimer’s and dementia initiatives. For more than two decades, he served on the public policy and advocacy staff of the Alzheimer’s Association, including as Director of State Policy and Advocacy Programs, where he led state and federal policy campaigns, trained grassroots advocates, and advanced priorities in Medicaid long-term care program reform, and comprehensive Alzheimer’s planning. His influence extends globally through collaborations with Alzheimer’s Disease International (ADI), the World Health Organization (WHO), and other partners to shape national and international dementia strategies.
As founder of Splaine Consulting and Managing Partner of Recruitment Partners LLC, Michael helps organizations, research teams, and policymakers achieve measurable impact through advocacy, strategic planning, and innovative approaches. His work with Recruitment Partners has improved enrollment in national and international dementia studies by engaging underrepresented communities where they live and receive services. Known worldwide for bringing the real face of Alzheimer’s into the policy arena, Michael continues to drive change that benefits people living with dementia and their care partners.
Solo Aging by Design: Building Belonging, Planning Ahead, & Strengthening Community
As our population ages, more adults are navigating life without traditional family structures - and many of us are still learning how to support them well. This session is for professionals who want to move beyond crisis-driven thinking and help solo agers build lives that feel secure, connected, and genuinely theirs. We'll explore practical ways to shift from "who will take care of you?" to "how can we help you build what you need?"
Three Key Takeaways:
The Support Network Audit: A practical tool to move beyond the "one primary contact" model. Learn how to help clients map a "Circle of Care" that distributes responsibilities across a network, surfacing gaps before they become emergencies.
The Digital Access Guardrails: Strategies for "crisis-proofing" a solo ager's digital life. We’ll cover the essential access points - from Legacy Contacts to 2FA recovery- that allow a support network to actually step in and help when it matters most.
Reframing the "Ask": How to help clients overcome the resistance to receiving help. You’ll leave with strategies to help solo agers navigate and reframe proactive planning as a gift of clarity to their chosen support system.
About the Speaker
Marni Blank is a lawyer turned end-of-life & after loss consultant. She founded Begin With The End, helping individuals and families prepare for, and move through, the emotional and logistical realities of death and grief. Her work blends practical support with deep compassion, helping clients complete end-of-life plans, navigate family dynamics, create meaningful legacy projects, and manage the administrative tasks that follow a loss.
Marni holds a BA from the University of Rochester and a JD from Brooklyn Law School. She completed her end-of-life doula training through Going with Grace and INELDA Bridge Training Program, mediation training through New York Peace Institute and Elder Decisions, grief training with Living Through Loss Counselling Society and From Grief to Gratitude Certification Program, and is NEDA proficient. She also completed her After Loss training through PALS (Professional After Loss Services). She splits her time between Brooklyn and upstate New York, finding balance between the energy of city life and the quiet of the mountains. Her snuggly (and mischievous) labradoodle Penny keeps her company.
Standardizing Home Modifications: How to Identify, track, and Measure Results
About the Speakers
Carolyn Sithong has over 20 years of experience as a clinical, educational and community-based occupational therapist with award winning practices. She is specially certified in the area of environmental modifications (SCEM) by the American Occupational Therapy Association (AOTA). Carolyn has been awarded “CAPS Designee of the Year” by the National Association of Home Builders and named on the roster of fellows at AOTA for her role in advocacy and leadership.
She has spear headed aging in place initiatives and home modification programming for local, state and national organizations that support healthy environments for the aging community. She has served the Florida Department of Elder Affairs, The Florida Association of Centers for Independent Living, NC Statewide Independent Living Council, AARP, National Rebuilding Together, Home Depot and AOTA, with her expertise in the home modification area. She is the current CEO of Home for Life Design.
Cindi Petito has practiced as an occupational therapist for 30 years in multiple healthcare settings, treating aging adults and individuals with traumatic brain and spinal cord injuries, and progressive neurodegenerative diseases. She has 26 years of experience working in complex rehab technologies (CRT) including wheeled mobility and custom seating, home accessibility products and home modifications, and various assistive technologies such as smart home technologies.

Cadense helps people walk more safely, confidently, and naturally — especially when walking feels difficult or uncertain.
They have design adaptive footwear for people who experience challenges like toe drag, instability, hesitation, or fatigue while walking. Their shoes are built to reduce trips and falls and help each step feel smoother and more controlled.
Cadense began with research at MIT where founder of Cadense, Dr. Tyler Susko, worked on the MIT Skywalker, a robotic gait rehabilitation system designed to help people walk more rhythmically after neurological injury.
